Take a soil sample from 1 inch below the top of the ground.

Take 4 samples from different locations and combine them into a packet for a more accurate result.

Sample need not be more than 500 grams. For that matter, 200 gms is adequate.

After you collect soil samples from 4 different locations, mix them thoroughly. Put 200 gms in a strong packet and dispose of the rest.
